You have the option to select one of two medical plan options offered through Carrier.

The basic features of the medical plans are highlighted below. A comparison of some of the key benefits provided by each plan is on the next page. You may use this information to help you choose the medical plan that is best for you. Regardless of the medical plan you choose, you will have access to the Carrier XYZ Network.

XYZ Network

Under this plan, only in-network services are covered. However, Carrier offers an extensive network of physicians and hospitals so you can choose the right in-network doctor or hospital for you. Keep in mind that any services received out-of-network will not be covered under this plan.

XYZ Network

Under this plan, you have the option of choosing any doctor or facility you want—whether in-network or out-of-network, to receive covered services.
However, keep in mind that you will typically pay less out of pocket for services you receive in-network. This plan has lower in-network deductibles and copays than the Blue Plan when you utilize services but you’ll pay a higher contribution per paycheck.
